Double-plating for comminuted osteoporotic proximal humerus fracture

Abstract

Introduction: Some proximal humerus fractures in elderly patients with osteoporosis are accompanied by medial metaphyseal comminution. Lateral Philos plate fixation is the most commonly used treatment. However, it cannot prevent loss of medial reduction and varus deformity, especially in cases of an unstable medial column.
 
Objective: To demonstrate that double-plating for proximal humerus fractures increases fixation stability.
 
Case report: We report the case of three octogenarian female patients with Neer type 3 comminuted osteoporotic proximal humerus fractures, severe varus displacement, and medial column involvement. They were treated with double plating, and satisfactory functional and radiological results were achieved.

Conclusions: Medial buttress plate fixation, in conjunction with the lateral Philos plate, is a viable option for the treatment of proximal humerus fractures in patients with an unstable medial column, varus deformity, and osteoporosis.

Biography

Dr. Renán Estuardo Vargas Morales is an Orthopedic Surgeon and Traumatologist since 2006. He holds a Doctorate in Planning and Management and a Master's in University Teaching. He is a Postgraduate Lecturer in Orthopedics and Traumatology at the Private University Antenor Orrego. He won the research contest at the International Congress of the Medical College of Peru (2001). He is a Full Member of the Peruvian Society of Orthopedics and Traumatology, and a speaker at international congresses of the Peruvian Society of Orthopedics and Traumatology and the International Society of Pelvis and Acetabulum. He is also an active member of the Association for Osteosynthesis Trauma and recognized as a Peruvian Researcher with RENACYT Code P0010700. Additionally, he is a peer reviewer for Orthopedics and Traumatology journals in Cuba and Colombia. He completed fellowships in the Knee Unit (Spain, 2008) and Musculoskeletal Trauma (Colombia, 2006). He has authored several publications in journals indexed in Scopus and Scielo.
